Please note, the damaged passport will not be returned to you.

Use this procedure if:

• You are between 16-18 years of age
  
• Your passport's binding is coming apart
  
• The information page of your passport is damaged (photo is not clearly visible, barcode is damaged)
  
• Your passport was washed with laundry, chewed by a pet, used as a coloring book, etc.
  

STEP 2: E-MAIL DOCUMENTS FOR REVIEW- DO NOT SKIP THIS STEP!

Our passport specialists will e-mail your final application packet with instructions on where to go, how much to pay and how to track your application.

STEP 3: RECEIVE FINAL APPLICATION PACKET

COVID-19 UPDATE:  AS OF 09/28/2020 US STATE DEPARTMENT RESUMED EXPEDITED PROCESSING FOR PASSPORT APPLICATIONS. ALL APPLICATIONS MUST BE SENT DIRECTLY TO THE GOVERNMENT FOR PROCESSING. IT WILL TAKE 4-6 WEEKS TO COMPLETE.

WE CAN HELP YOU PREPARE ALL PAPERWORK TO HELP MINIMIZE A CHANCE OF MISTAKES AND REQUESTS FOR ADDITIONAL DOCUMENTS/INFORMATION. FOLLOW THE STEPS BELOW TO START PROCESSING YOUR PASSPORT NOW USING EXPEDITED PROCESS. YOU WILL BE MAILING YOUR PAPERWORK DIRECTLY TO THE GOVERNMENT FROM THE POST OFFICE. AMERICAN VISA SERVICE IS ONLY ABLE TO ASSIST WITH DOCUMENT PREPARATION. PLEASE NOTE: EXPEDITED PROCESSING WILL STILL TAKE 4-6 WEEKS.

*Please note that American Visa Service will not be able to get involved after your original documents are mailed to the National Passport Processing Center or contact the Passport Processing Center on your behalf. If additional documents or information is requested by the government and you need assistance in understanding the communications, putting together additional paperwork and submitting, additional fees will apply.

FEES

• Copy of US birth certificate with both parents' names or Certificate of Naturalization
  
• Copy of damaged passport (it will not be returned to you)
  
• Two passport size photos 2"X 2"  (must be taken within last 2-3 months), cannot wear glasses
  
• One Application DS-11 (click here to download)
  
• Statement from the applicant explaining how the passport was damaged
  
• Copy of applicant's driver's license or student ID (front & back)
  
• Copy of parent's ID (front & back) who will go to the post office with the teen (for applicants who are 16 or 17 old)
